Reason for visit

‘Reason for visit’ is rarely recorded in GP clinical information systems – that is, it is left blank in the patient health record. This can mean that there is insufficient information for clinical handover and clinicians may miss important information, resulting in poor continuity of care.

The settings of the clinical information system can be changed to ensure the user receives a prompt to enter a ‘Reason for visit’ when closing a patient health record. This is something a practice manager or systems administrator can manage.
