Past medical history

‘Past medical history’, also called ‘Problem list’ in some clinical information systems, can be full of redundant data. When ‘Past medical history’ is not used correctly, it can result in:

  • Failure to treat or to follow best practice guidelines
  • Failure to consider relevant surgical history or comorbidities.

To avoid these issues, it is important to only enter chronic conditions and significant events in ‘Past medical history’. This is something that a practice manager or systems administrator can set up through the clinical information system’s settings/preferences.
